Transforming End-of-Life Conversations One Card at a Time
By Ashley Johnson
Death is one of the few experiences every person will face, yet it remains one of the most avoided conversations in our society. My journey as an end-of-life doula began with a simple but profound realization: many people are unprepared for the emotional, practical, and relational decisions that accompany serious illness, dying, and death, not because they do not care, but because they do not know how to start the conversation.

As an end-of-life doula, researcher, TEDx speaker, and healthcare advocate, I have dedicated my career to helping individuals and families navigate some of life's most meaningful transitions. Through my work, I have witnessed firsthand how silence around death can create confusion, conflict, and missed opportunities for connection. I have also seen how a single conversation can bring clarity, healing, and peace.
That understanding inspired me to create an end-of-life conversational card deck designed to help people engage in discussions that are often difficult but deeply important.
The card deck serves as a bridge between uncertainty and understanding, offering thoughtful prompts that encourage individuals, families, caregivers, and communities to explore their values, wishes, fears, and priorities before a crisis occurs.
What makes these conversations so powerful is that they are not solely about death. They are about how we want to live, how we want to be cared for, what matters most to us, and how we want to be remembered. The card deck creates a safe and approachable way to discuss topics such as advance care planning, legacy, relationships, spirituality, caregiving, and personal preferences. By transforming intimidating conversations into guided dialogue, people can engage with greater confidence and compassion.
My work is rooted in a commitment to health equity and accessibility. Throughout my career, I have focused on ensuring that underserved and historically marginalized communities have access to end-of-life education and resources. Too often, conversations about death and dying happen too late or not at all. I believe everyone deserves the opportunity to make informed choices and have their voice heard.
The response to the card deck has reinforced what I have long believed: people are not unwilling to discuss end-of-life issues, they simply need the right tools and supportive environment to begin. Families have shared stories of deeper connections, reduced anxiety, and meaningful discussions that may never have happened otherwise.
As our society continues to evolve in how we approach aging, caregiving, and serious illness, I believe we have an opportunity to normalize conversations about mortality. When we do, we create space for greater understanding, intentional planning, and compassionate care.

My mission is simple: to help people move from avoidance to empowerment. Whether through education, advocacy, community engagement, or innovative tools like the conversational card deck, I am committed to ensuring that end-of-life conversations become less fearful and more human.
Because when we talk openly about what matters most, we don't just prepare for death, we enrich the way we live.
